Name Change

To update the name on your license, you will need to fill out the Personal Name Change form and send it, along with a legal document showing your name change, to [email protected]. 

It can take up to four weeks for your name to be updated. Once it is updated, you will receive an email.

• Log in to the portal. If you haven't created an account in the new portal, you can do this by selecting the 'Sign up Now' link. 
• Select 'Name Change' in the left navigation bar.
  • You will need to upload a legal document showing the name change.
