Records Reproduction Request

A records reproduction request is if you are needing a comprehensive list of licensee information in a designated area or statewide. For example: A salon owner is opening a new building with salon suites in Phoenix and wants to market to all cosmetologist licensees in Phoenix. 

Please note that the Arizona Barbering & Cosmetology Board charges for Records Reproduction Requests. Records provided include the license number, first and last name, mailing address, and email address.

If you are submitting a request for a commercial purpose, you must disclose this pursuant to A.R.S. 39-121.03. Please note that a commercial usage fee does apply.
